Understanding Your Health Insurance

Here are some common insurance terms that can help you better understand your coverage:

Allowed Amount
The maximum amount your insurance will pay for a covered healthcare service.

Co-Insurance
Your share of the cost for a covered service, usually calculated as a percentage of the allowed amount. You pay this plus any deductible.

Co-Payment (Copay)
A fixed amount you pay for a covered healthcare service—often due at the time of your appointment.

Deductible
The amount you owe for covered healthcare services before your insurance begins to pay. This may not apply to all services.

Out-of-Pocket Limit
The most you will pay during a plan year before your insurance covers 100% of allowed amounts. This typically excludes your premium and services not covered by your plan.

Prior Authorizations
Certain services, medications, or procedures may require approval from your insurance before they can be performed. This is common for biologics and infusions. Our office will always notify you if prior authorization is required.

Referral
Some insurance plans require a valid referral from your primary care provider before you can see a specialist. Our office verifies all referrals before your appointment. If your referral is not active or authorized, we may ask you to reschedule or self-pay for your visit.

Self-Pay
If you are uninsured or opt not to use your insurance, you may qualify for our self-pay rates. These rates must be arranged in advance of your visit.
